Support Coordination


In the role of Support Coordinator, you will assist National Disability Insurance Scheme (NDIS) participants to coordinate and implement a range of support services, in line with their NDIS plan.

You will also be required to establish strong rapport and working relationships with a range of stakeholders including People with Disability and their family members, NDIS Local Area Coordinators (LAC'S), NDIS Planners and internal and external service providers.

ESSENTIAL CRITERIA:

  • Excellent verbal communication skills and a demonstrated ability to engage with clients and their families to support them as participants of the NDIS
  • Strong written and computer skills for the creation of service agreements, reports, correspondence and other documents
  • Understanding of the National Disability Insurance Scheme
  • Experience in writing clear, concise and informative reports
  • Demonstrated knowledge of Client Directed Care principles and frameworks
  • Goal orientated
  • High level of confidentiality, self-motivation and ethics
  • Demonstrated experience in working with people with complex needs, including supporting participants to develop positive relationships with relevant stakeholders
  • Established time management, problem solving and conflict management skills
  • Ability to think creatively and use initiative, to work independently and as part of a multidisciplinary team using strong coordination, leadership and peer support skills
  • Knowledge of community based support services
  • Manage time, resources, task and competing priorities
  • Work independently and respond to a rapidly changing work environment
  • Knowledge of Disability Services Standards; Disability Services Act; Cultural Diversity; EEO; WHS and Mental Health Standards
  • Full employment history, including level and year you completed your education
Licences and checks needed:
  • Current drivers license, access to a reliable and comprehensively insured motor vehicle
  • Current RMS Driving Record (to be provided with application)
  • Satisfactory National Police Check-less than 6 months old (to be provided with application)
  • Working With Children Check (to be provided with application)
  • NDIS (National Disability Insurance Scheme) Worker Orientation Certificate. This is mandatory prior to commencement. www.ndiscommission.gov.au/workers/training-course
  • NDIS Worker Clearance Screening – All successful applicants are required to obtain this clearance prior to commencement of the role. This is a mandatory clearance for the disability industry. More information can be found at Services NSW or you can speak to our HR team on 63801601

DESIRABLE CRITERIA:

  • Relevant tertiary qualifications or studying towards qualifications which may include Social Welfare, Mental Health, Social Work etc
  • Minimum Certificate III or higher in Disability or Mental Health and/or equivalent experience
  • Experience in providing support coordination under the NDIS
  • Experience in complex case management within the disability field

Conditions of Employment:

  • As per Social, Community, Home Care and Disability Services Industry Award 2010
  • Full time and PPT considered, subject to completion of satisfactory 6 month trial
