An exciting opportunity to join the Acute Post Acute Care Team (APAC) here in the beautiful Central Coast as a Nurse Coordinator (CNS2) 0.84 FTE, 7 day rotating rosters.

The Nurse Coordinator is responsible for the provision of safe high quality patient centered nursing care to patients requiring Hospital in the Home services. This outcome will be achieved through the exercise of advanced nursing skills, professional knowledge and judgment to provide a high standard of assessment, clinical care, expert advice and educational support to staff, patients, family and carers.

What you'll be doing The Nurse Coordinator is responsible for the provision of safe high quality patient centred nursing care to patients requiring Hospital in the Home services. This outcome will be achieved through the exercise of advanced nursing skills, professional knowledge and judgement to provide a high standard of assessment, clinical care, expert advice and educational support to staff, patients, family and carers.

Consistent with the strategic directions of Central Coast Local Health District (CCLHD) ‘Caring for the Coast’ and service plans, the Clinical Coordinator has a leadership role in the development, provision of evidence based nursing clinical practice related to Hospital in the Home. While working collaboratively with the multidisciplinary team, the Nurse Coordinator will act as a clinical expert, professional role model and change agent to promote practice development and quality initiatives that promote the delivery of safe, efficient and high quality care to patients requiring Hospital in the Home. The Nurse Coordinator will support timely access and appropriate transfer of care in collaboration with the multidisciplinary teams involved with the care of the patient.

As an employee of CCLHD, you will be rostered in accordance with the Nurses and Midwives (State) Award and in line with the operational requirements of your unit/ward/service.
