What you'll be doing

  • This position is responsible for providing culturally sensitive support to pregnant women and their families during pregnancy, facilitating informal support groups and facilitating access to appropriate services.
  • This position also works towards improving health outcomes and wellbeing of Aboriginal families on the Central Coast with young children (0-5 years of age) by providing culturally sensitive care navigation for the family.
  • Care is provided via telephone, telehealth where available, home visits and hospital and/or community settings.
  • The Aboriginal Health Worker works in partnership with the family and with Registered Midwives in the antenatal and post- natal period, and Child and Family Health Nurses up to the child reaching school age for Aboriginal women and women whose babies identify as Aboriginal and/or Torres Strait Islander as per Ngiyang Aboriginal Pregnancy, Child & Family Health Service Model of Care.
