About the program

Better Futures is a new approach to improve outcomes for young people transitioning from care into independence. The program aims to provide young people support across five service offers; housing, education, employment, health and wellbeing and community connections. Eligible young people are referred at 15.9 month and have access to support until their 21st birthday.

About the role

The Better Futures Case Manager will be an integral part of the young person’s care team while in care and continue to provide ongoing tailored support to help achieve their goals and aspirations once they have transitioned from care.

Each young person will be offered a level of support and this will be dependent upon the circumstances of the young person and capacity as established through intake. The Better Futures Case Manager will be required to work with young people in all levels of support.

Providing flexible, holistic and tailored support in line with the Advantage Thinking Framework.
Develop and support eligible young people with individual support plans as per the Better Futures guidelines.
Work within the required funding framework, program guidelines and to comply with statutory requirements where required.
Accurately complete and maintain all necessary records, reports, case notes and outcomes in accordance with the funding contracts and best practice guidelines.
To administer flexible funding as approved in line with the Better Futures guidelines.
Skills and experience
A degree in Youth Work, Social Work, Community Development or similar.
Demonstrated case management experience, preferably within the youth sector.
Demonstrated capacity to provide effective case management support to young people in out of home care services specifically kinship, foster care, residential and lead tenant services.
Knowledge of the issues that contribute to trauma and disengagement experienced by young people in out of home care.
Demonstrated knowledge and experience working within a care team with key community agencies and DFFH services such as Child Protection to support current and previous statutory young people who require a range of specialist support.
Knowledge of DFFH systems, legislation and ability to comply with DFFH standards.
