Reporting to the General Manager of Care Income and Support, you will have a portfolio and be responsible for supporting residents and their family’s through their first 28 days of their journey into residential care.

We are seeking a motivated, committed and passionate person who can work autonomously, show leadership skills and work well as part of a team.

You will be responsible for maximising our funding by ensuring excellent documentation and well written assessments and care plans to ensure Royal Freemasons residents get the best funding outcomes so they can continue to live the life they deserve.

Key duties:

  • Perform audits and provide support to the homes during ACFI (funding tool) validations
  • Work with Residential Support teams to assist with reviewing/monitoring of ACFI (funding tool)
  • Assist with completing assessments / care plans by capturing all assessed care needs identified
  • Oversee the development, implementation and evaluation of assessments and care plans in consultation with the care team, residents, and their families
  • Develop and design assessments and care plans that are reflective of residents needs and improves the outcome for residents
  • Attend Resident of the Day (ROD) meetings in consultation with the Care Manager and all care staff
  • Conduct all admissions
  • Work with the ACFI and Documentation Manager to develop a standardized approach to ACFI documentation
  • Provide support and education to Managers and support staff
  • Work alongside quality and Innovation Team with identifying and providing feedback on education programs or services


  • Tertiary qualification in a relevant discipline Registered RN or EEN (AHPRA registration)
  • A current driver’s licence with no restrictions


  • Ability to work independently, to plan, coordinate, implement, communicate, and evaluate funding and quality improvements, through education of staff
  • Excellent communication, both written and oral and interpersonal skills
  • Computer literacy to operate E-case documentation systems, MS office


  • Minimum 3 years’ experience in the entire ACFI (funding tool) process from education and direction of staff, to the final overall checking and assessing prior to submission.
  • Strong understanding and experience of the aged care funding instruments business rules
  • Strong understanding and experience in the validation of ACFI packs by the department


  • Working knowledge of the Aged Care Act and accountability Principles and other relevant legislation.
  • Strong understanding the new aged care accreditation standards
  • An understanding of legislative and regulatory requirements relative to the role
