This is a Fixed Term Part Time appointment until 30/06/2022 at 45.6 hours per fortnight.

About The Position

WA state-wide Training Centre in Subacute Care WA (TRACS WA) is seeking to appoint a Development Facilitator. The candidate must have substantial and recent work experience in the subacute care domain.

In this role, you will work in consultation with TRACS WA Clinical Leads and the Unit Coordinator to develop and facilitate subacute care training programs as per the clinical domains identified in the WA Subacute Care Plan. You will provide support and consultation to the Subacute Care Community of Practice. You will also develop educational content for website, technological advancements and other projects as directed by the Unit Coordinator.

TRACS WA is a training unit working in partnership with the Department of Health WA and the WA Primary Health Alliance. TRACS WA is the Western Australian Department of Health's training unit specialising in subacute care. We were established in April 2012 as a key recommendation of the WA Subacute Care Plan 2009-2013.

Our aim is to improve the quality of health services provided to Western Australians in subacute care areas such as rehabilitation and older adult mental health.

This is achieved by providing targeted training and skills development opportunities for staff who work in subacute care and by developing and supporting communities of practice around specific clinical areas. We also support the development of activities or projects that address service gaps in subacute care, however we are no longer able to provide funding support.
