JOB DESCRIPTION

Our partner, a leading home and community care organisation based in Mount Waverley is seeking an immediate need of a Care Coordinator for an 8 week period. The incumbent needs a strong disability background &/or clinic skills.

As a Care Coordinator, You Will Support Complex Care Coordination And Planning To Clients Across Various Funding Streams (Home Care Packages Guidelines, Individual Support Packages, HACC, And The Disability Standards), And You Will Be Responsible For Developing Partnerships With Clients To Proactively Achieve "solution-focused" Outcomes, Including

  • Client assessments
  • Support plans
  • Rostering care; and
  • Providing services to meet the client's needs.
You will work to develop positive working relationships within our local service center and more broadly across the organisation to facilitate a streamlined client experience.

Care Coordinators work with Support Workers to promote person-centred and re-enablement focused care and monitor compliance service delivery.

Responsibilities
  • Completion of care coordination activities for community clients in collaboration with the Case Manager or Service Coordinator.
  • Incorporating innovative approaches to meet the needs of the clients.
  • Respond promptly and appropriately to all care coordination activities, according to organisation policies and procedures. This may involve:
i) Participating in the induction of newly employed Support Workers;

ii) Monitoring Support Worker compliance with policies and procedures and escalating issues to the Service Coordinator;

iii) Monitoring Support Worker compliance with service delivery and escalating issues to the Service Coordinator;

iv) Receiving and actioning feedback from Support Workers in relation to rostering and escalating as required;

v) Identifying WHS issues and actioning/escalating as per organisation policies and procedures;

vi) Preparing and updating client schedules of care and task lists in collaboration with the client;

vii) Direct communication with clients and their carer/representative in relation to satisfaction with service delivery and escalating complaints in accordance with the organisation’s policies and procedures;

viii) Participating in reconciling accounts in line with the client’s budget (as indicated);

ix) Providing information about Calvary Community Care and other community service options that could assist clients.
  • Assist with the onboarding of external brokerage agencies.
  • Complete data entry within the client management system according to organisation policies and procedures.
  • Liaise with and develop positive working relationships with other professionals and agencies in the area, participating in relevant forums and meetings as indicated.
  • Attend meetings as required and contribute towards service improvements as a member of the Calvary Community Care team.
  • Understand and use Microsoft Office packages and develop expertise in using Calvary Community Care’s database and email systems.
  • Work within a budget framework and prepare data for reporting as required.
  • Comply and work within all policies, procedures, and guidelines of Calvary Community Care and of the funding body.
  • Assist with confirmation and processing of invoices.
  • Other duties as directed.
