    About the role

    We are thrilled to be setting up Home Care Packages (HCP) as a service offering. The role on offer is HOme Care Specialist and this role forms a critical part of starting up our Home Care Package business. HCP’s aim to meet a wide range of needs, by coordinating a mix of services that can help seniors in the community live independently.

    As a Home Care Specialist, you will engage with customers throughout the Home Care journey helping them to understand the variety of support services available to meet their individual needs. It is an important and diverse role

    This is a Permanent Full-time position, which has flexibility to be based on the Central Coast or in Northern Sydney. Flexibility is required to travel to other locations within the Diocese of Broken Bay.

    This is an exciting new start-up opportunity in our organisation and will initially be a dual role (Home Care Specialist and Community Engagement Officer).

    Duties

    • An active participant in the HCP Implementation Project
    • Customer Service focus, ensuring client’s feel supported and informed.
    • Undertake initial Client Assessments and attend initial “Face to Face” visits with client in their home.
    • Networking with internal and external Aged Care related programs to grow and maintain referral pathways.

    Skills & experience

    • Minimum Diploma level qualification in related field.
    • Work experience in the Home Care Sector. (5 years experience is desirable)
    • Exceptional interpersonal and rapport building skills, with a solution focus approach.
    • Ability to work independently with initiative and collaboration where required.
    • Aptitude for understanding new systems and technologies, particularly the My Aged Care system.
    • Current driver's license and access to insured vehicle.
