The Organisation

We’ve partnered with one of Victoria’s leading community based, not-for-profit, aged care providers to find them a new part-time Home Care Coordinator. With sites throughout Melbourne, this role will be based in Pascoe Vale and will be 3 full days per week. We have an immediate start opportunity with the potential to also go permanent.

The Role

  • Case management with comprehensive assessment, planning and evaluation
  • Working with residents to enable them to make choices on their lifestyle
  • Deliver and monitor service provision within budget requirements
  • Lead, motivate and mentor junior members of staff
  • Provide clinical care as provided
  • Ensure client records are kept up to date

What you need
  • Nursing or Allied Health degree qualification
  • Current Nursing registration with APHRA
  • Prior Case Management experience
  • Strong assessment skills and ability to problem solve complex cases
  • Great interpersonal and rapport building skills
  • Current driver's license

On Offer

Great work-life balance on offer with this three days per week part time opportunity based in Pascoe Vale. Temporary or permanent option available at an hourly rate of up to $42 /hour plus super.
