About the role

The ASERT Clinician is responsible for working with treating teams, assisting in the effective and timely assessment of patients and supporting the referral and discharge of patients in consultation with the treating teams. The role is responsible for;

? Developing and promoting strategies that streamline and improve the discharge planning process.

? Promoting and encouraging safe and efficient discharge and utilisation of discharge alternatives where appropriate.

? Ensuring a smooth and efficient transition from hospital to home for patients with post discharge service requirements.

? Facilitating patient flow and aiming to achieve the most timely and positive patient outcomes.

? Assisting in reducing length of stay and readmission by providing comprehensive discharge planning support.

? Implementation of access and patient flow initiatives across Eastern Health.

? Undertakes provision of care Coordination for patients requiring Post-Acute Care Services

About you

To be successful in this role you will possess

  • Highly developed assessment skills to determine patient discharge needs
  • Discharge planning skills
  • Working knowledge of Ambulatory and community services
  • Demonstrated negotiation, problem solving and analytical skills.
  • Demonstrated ability to implement innovative practice.
  • Demonstrated record of achievement of quality service delivery.
  • Current registration with AHPRA or affliliated organisation
  • Post Graduate qualifications in area of speciality
  • Highly developed interpersonal and leadership experience
  • A strong commitment to high quality care
  • The ability to work as part of a diverse team
  • Excellent communication and organisational skills
