About the Role

  • In collaboration with the Renal Supportive Care team, the Renal Supportive Care (RSC) Social Worker will help to identify the psychosocial needs of families/carers using the service. The RSC Social Worker will provide a social work service to referred renal patients and their carers, and also act as a consultant on psychosocial issues to the RSC team.
  • The RSC Social Worker will provide a range of social work interventions, to ensure continuity of care, safety, and optimal outcomes for patients and their carers.

Selection Criteria to be addressed:

  1. A Social Work Degree accredited by the Australian Association of Social Workers
  2. Advanced reasoning and practice skills, an ability to operate autonomously with minimal direct clinical supervision and a capacity to apply professional knowledge and judgement when performing novel, complex or critical social work tasks
  3. Practise with a high level of cultural competence, including a sound knowledge of issues affecting the people of First Nations.
  4. Significant experience in a health setting and extensive knowledge of the psychosocial consequences of illness
  5. Experience in end of life care
  6. Ability to build effective relationships with relevant government, non-government and community organisations
  7. High level written and verbal communication skills
  8. An understanding of child protection principles and statutory requirements
