  • Total Indicative Remuneration: $79,988 - $83,412 p.a. (pro rata) - OPS4 - Temporary up to 31 December 2021 - Part Time - 22.5 hours per week
As the Aboriginal and Torres Strait Islander Health Practitioner-Foot Champion, you will work within NALHN as a key member of the Northern Model of Care: Prevention and Management of Diabetic Foot Disease in Aboriginal and Torres Strait Islander People NALHN Project. You will work in collaboration with the Senior Podiatrist Aboriginal Health NALHN and Watto Purrunna NALHN Aboriginal Health.

In this role, you will assist in implementing and embedding the Northern Model of Care: Prevention and Management of Diabetic Foot Disease in Aboriginal and Torres Strait Islander People NALHN Project a stronger and more sustainable approach to support of Aboriginal clients in the primary healthcare setting, through providing an ‘Aboriginal lens' to health care and delivering cultural safe care. In collaboration with the multidisciplinary teams you will assist in assessment and providing clinical intervention to Aboriginal clients, including those with complex needs. You will assist multi-disciplinary team members on culturally appropriate interventions, care planning and education within their scope of practice. The service you provide will aim to reduce the risk of Aboriginal clients disengaging with treatment and care plans. You will provide acute and sub-acute health responses, support hospital avoidance and advise on length of stay in the acute sector and closer to home services.

You must hold,
  • Certificate IV Aboriginal & Torres Strait Islander Primary Health Care (Practice) or equivalent.
  • Registration with the Aboriginal and Torres Strait Islander Health Practice Board of Australia.
Appointment will be subject to a satisfactory Criminal History Check.
