Registered Nurse - Integrated Chronic Care for Aboriginal People Program

About the role:
Tamworth Community Health is seeking a motivated Registered Nurse to provide compassionate person centred care within the Aboriginal Health Unit as part of the Integrated Chronic Care for Aboriginal People Program (ICCAPP), ensuring safe care is delivered in collaboration with patients, families, carers and a multidisciplinary team to achieve identified goals and outcomes. The Integrated Chronic Care for Aboriginal People Program (ICCAPP) is a community outreach service that aims to increase access to culturally appropriate health services for Aboriginal and Torres Strait Islander people aged 15 years and older, who are at risk of or who have a chronic disease.

In this role you will work in partnership with Aboriginal chronic care patients, families, carers and the community using a culturally sensitive and individualised patient centred approach to provide the necessary cultural support, care and advice. Networking and education opportunities are offered locally and through Aboriginal Health Unit to the successful applicant.

About ICCAP:
This position is based within the ICCAPP in the Aboriginal Health Unit at Tamworth Community Health. Our staff includes a Clinical Nurse Consultant, an Aboriginal Health Practitioner and a Registered Nurse. Our Unit focuses on maintaining the health and wellness of Aboriginal people in the community including those with chronic disease and those recently discharged from hospital. The program aims to provide a practical approach to improving health outcomes for Aboriginal people with or at risk of developing a chronic disease by working in partnership to develop and support flexible local approaches to chronic disease.

About you:
You are a dedicated Registered Nurse with a demonstrated commitment to improving Aboriginal Health outcomes and good community connections. You will call upon your strong relationships and ties with key stakeholders and ability to liaise with a range of community clients and organisations/stakeholders, along with your chronic disease knowledge, understanding and/or experience to ensure a supportive, flexible and individualised approach to care is given to all patients.
