About Department

This position is part of the Health Independence Program (HIP). The intent of HIP is to deliver improved outcomes for clients and to support hospital demand management and flow, through delivering integrated and coordinated care to clients across the hospital and community interface, reducing duplication and fragmentation of services and ensuring client centred care planning.

SVHM HIP consists of integrated hub sites across several locations including in Fitzroy, Kew, North Richmond and Fairfield.

This position will be part of the HIP Central Intake Team based in the first instance at the Fitzroy site.

This position may be required to work at any of the HIP hub sites or programs. This will be at the discretion of the HIP manager and will take into consideration appropriate skills and training.

The key aim of HIP Central Intake Team is to provide an access point for HIP Services. Referral processing, initial needs assessment and recruitment to programs are the main functions of the HIP Central Team.

HIP at SVHM is delivered in partnership with North Richmond Community Health (NRCH).

About the Role

An opportunity currently exists for an Administration Assistant to join the Health Independence Program (HIP) Central Intake team on a permanent basis.

This dynamic role will be responsible for a broad range of administrative functions that contribute to the successful achievement of HIP key performance indicators and service delivery including referral processing, answering telephone queries, appointment scheduling and maintenance of client database systems and medical records. The Administration Assistant provides positive, pro-active and practical support to staff, teams and managers, to assist in the provision of high quality and effective clinical care services to HIP consumers.

Classification for this position is HS1 ($58,442 per annum)

Duties
  • Work under the direction and in support of the HIP Central Team Leader and HIP Stream Manager
  • Maintain the patient registration systems and databases in a timely manner, ensuring all details are accurate.
  • Assist with the day to day HIP operational processes, including answering the telephone, assisting clinical staff, processing referrals and making appointments.
  • Communicate, collaborate and consult with others appropriate to administrative roles and responsibilities, ensuring effective communication with clients and their families/carers, referrers and community agencies.
  • Be flexible and ready to provide support as directed by manager and team leader
Qualifications, Skills and Experience
  • Demonstrated experience working across a broad range of administrative functions and duties, within a healthcare setting.
  • Advanced customer service and interpersonal skills.
  • Effective communication skills (including verbal, written and skills in adapting communication styles to suit clients, families and/or carers, external service providers, team members and managers).
  • Advanced skills in Microsoft Office systems, including proficient typing skills.
  • Experience utilising patient management software systems such as Patient Administration System (PAS) and electronic medical records.
