About the role

The HOPE Initiative provides support for people at the point of hospital discharge that are identified as at-risk of suicide and/or repeated intentional self-harm and aims to support consumers to address those factors that contribute to suicidal ideation and support consumers to progress towards their preferred future. The aim of the initiative is to prevent avoidable suicide attempts and death by suicide by providing the target cohort with assertive, tailored postvention and psychosocial support in the community for a period of up to three months post discharge from hospital. The program also supports the person’s carers and families to identify and build the protective factors that reduce the risk of suicide attempt/completed suicide. 

The Family Support Worker, in collaboration with other staff, will utilise relevant and evidence based models of suicide intervention and post-vention, primarily Solution Focused Brief Therapy and the Collaborative Recovery Model to:

  • Work in partnership with carers, families to provide high quality support and intervention in order to improve family functioning and outcomes for referred consumers and their family/ carer supports

  • Utilise the principles and practice of recovery, to effectively engage the person’s family/ carers in linkages to community support services for ongoing support, care and/ or treatment.

About you

To be successful in this role you will possess:

  • A tertiary mental health or related qualification (Nursing, Social Work, Welfare, Psychology, Occupational Therapy, Disability Services, Community Development, or related discipline)

  • A current Victorian driver’s licence is essential

  • Demonstrated experience in the mental health field, with a minimum 3 years in the mental health setting

  • The ability to work as part of a diverse team

  • Excellent communication and organisational skills
