JOB DESCRIPTION

The role of Team Leader provides an opportunity for a motivated person to make an important contribution to Queensland through supporting people with disability to access the NDIS and the supports they require to be active participants in their communities. As Team Leader, you will provide support and advice to Case Managers to assist people with disability to access the NDIS.

The Assessment and Referral Team works closely with potential new NDIS participants and key stakeholders including other government and non-government agencies, community organisations, and medical and allied health professionals state-wide to increase the number of NDIS participants in QLD.

People with lived experience of disability are strongly encouraged to apply.
Possession of a C class drivers licence.
Some intrastate travel will be required.

Key responsibilities of this role

  • Apply an end-to-end intensive case management approach to support people with disability to access the NDIS.
  • Lead and manage a team to ensure delivery of high-quality intake, triage and intensive case management services to potential NDIS participants.
  • Provide support to the Manager and Principal Team Leader in ensuring the team delivers high quality support services and appropriate service responses to assist people with disability to access the NDIS.
  • Co-ordinate and facilitate meetings with key local stakeholders, including other government departments, non-government service providers and specialist disability service practitioners in relation to ART.
  • Represent the department at appropriate networks and meetings, including presenting information at community and non-government provider forums and networks to further assist people with disability to access the NDIS.
  • Contribute to analysis, review and evaluation of project targets to inform future planning processes and strategies to meet the needs of people with disability including providing advice to Executive Director, Director and Project Manager.
  • Participate in project planning, monitoring and evaluation while managing complex human, financial, physical and information resources and systems.
