Chronic Disease Manager required for rural medical service in NSW. 12 month contract or Permanent role

The role
We are currently seeking a Chronic Disease Manager. You will be a Registered Nurse with chronic disease case management experience as well as staff management experience. You will be assisting in the day to day running of the practice. Your main role here will be to manage a team of around 9 staff, preparing them daily to go out into the community; Helping them to provide Primary Health Care to patients who have Chronic Disease living in the community. You will manage different aspect of the organisation from patient care to planning and implementing health programs.

Our client is a community funded aboriginal organisation; who pride themselves on being a part of the local community. This health service are firm believers in equal rights, they opened the facility in the hope of giving something back to the community. Services here include, but are not limited to; Public Health, Family Health, Child Health, Sexual Health, Dental, Drug and Alcohol, Mental health and GP Clinical Services.

The location
You will be located in rural NSW in a small town of around 2500 people, the town has its own airport and plenty of amenities such as; shopping, District Hospital, Youth Centre, salons and a local swimming pool. There are also activities like; tennis, lawn bowls and golf. There are plenty of restaurants to keep you entertained. And good educational opportunities for children

The package
As the successful applicant you will be entitled to:
• Salary $90,000 + Super
• Including Accommodation + Relocation. Accommodation consists of beautiful modern fully furnished houses; there is an option for a shared house or a single, which you will be able to bring your family to.
• Leave loading
• Travel expenses to meet the team prior to appointment.

The criteria
In order to be considered for the role you must meet the following criteria:
• Bachelor of Nursing
• General AHPRA Registration
• Chronic Disease Experience
• Proven management skills
• WWCC and Police check
• Exceptional written and verbal communication skills and a strong team player
• Valid driver's licence
