About the role
South West Healthcare's (SWH) Transition Care Program facilitates continuum of care for older people who have completed a hospital episode. Transition care is goal-oriented, time-limited and therapy-focused. It provides older people with a package of services that includes low intensity therapy such as physiotherapy and occupational therapy, as well as social work, nursing support or personal care. It seeks to enable older people to return home after a hospital stay rather than enter residential care prematurely.

If you are passionate, experienced, and committed clinician adept working with evidence-based methods, we'd love you to join our team. Eligible candidates must be registered nurse or allied health professional with relevant post graduate qualifications and/or experience.
This position is a maternity leave positon, 64 hours per fortnight until July 2022. The position commences in July 2021.

Employment Terms and Conditions will be in line with the Nurses and Midwives (Victorian Public Sector) (Single Interest Employers) Enterprise Agreement 2016-2020 & Allied Health Professionals (Victorian Public Health Sector) Single Interest Enterprise Agreement 2016-2020, with the appropriate classification Registered Nurse Grade 4A or Allied Health Professional Grade 3.
