About your new role

A true generalist role, you will have the opportunity to expand your skillset whilst working with a broad range of participants. You will be providing a person-centred approach to individuals living with a disability and their team to empower and support participants in the lifestyle of their choice. You will be maintaining accurate reports, notes, and records to meet organisational, and legislative requirements.

Program facilitators oversee support workers to ensure participants have person-centered support. They ensure the participants wellbeing is monitored and their NDIS goals and funding is being used effectively. You will do this by communicating with stakeholders, support workers and the participants to ensure they have a voice and a choice over their life and supports.

The successful candidate will possess strong professional and ethical values.

You may also be required to do on call manager shifts. Participants and staff can call if they require after hours emergencies.

Skills and experience required.

  • An understanding of the National Disability Insurance Scheme (NDIS).
  • Ability to network with stakeholders to achieve positive outcomes.
  • Excellent communication and interpersonal skills with proven ability to establish sound relationships with participants and peers.

Essential Criteria:

  • Current W.A. Drivers Licence and your own car.
  • Ability to travel within the Perth Metropolitan area as required.
  • NDIS Workers Screening
  • Current Basic First Aid certificate (inclusive of HLTAID001, HLTAID002 and HLTAID003)
  • Completion of the NDIS Workers Orientation Module


  • Completion of the Hand Hygiene Standard Theory Online Module https://www.hha.org.au/online-learning/complete-a-module
  • Completion of the NDS Infection control module https://www.nds.org.au/events-and-training/all-events-and-training/infection-prevention-control-3461
  • Completion of the Hepatitis C module


  • Completion of the First Response Fire Safety Training


  • New Worker – NDIS Module


  • Bachelor of Social Work or Master Social Work
  • Registration with Australian Association of Social Workers

Desirable Criteria:

· Current Working with Children Check

· Cert 4 in disability or community services.
