Are you a highly organised professional, with a commitment to exceptional, person centered care? Join a group of likeminded, dedicated individuals assisting our clients to remain living independently in their own homes as they age.

The role

Due to an internal promotion, we are seeking someone experienced in community care to work collaboratively with ECH clients, their carers, families and advocates helping them navigate through the aged care system and ECH’s model of care. Care Coordinators ensure ECH delivers quality services that promote self-determination which are individually tailored to the service needs of the individual client, enabling each of them to have the best life possible as they age.

This is a highly mobile role, with the majority of your time spent out of office visiting the clients in your case portfolio.

What you’ll be doing

Under the guidance of our operational and clinical service leadership team, our Care Coordinators:

Provide exemplary customer service as the primary point of contact for our clients and their advocate(s);
Promote ECH, develop effective networks to support ECH business growth and targets whilst promoting ECH services and products that enable clients to achieve their goals.
Oversee a case load of Home Care Package clients
Work to sign up new Home Care Package clients
Provide advice and guidance to potential and existing clients to enable informed decision making and self-determination
Undertake comprehensive assessments of client needs to ensure that individualised, goals are developed to enable clients to continue to live in the community.
Identify when a client’s needs have changed, provide a timely response and amend services to account for the client’s changing situation to maximise clients independence.

You will bring to the team

A demonstrated genuine commitment to helping older ageing people retain their independence so they can remain at home in the community
Relevant tertiary qualifications in health, social or behavioural sciences, and /or extensive care coordination experience
Previous experience with client care in the health & community sector.
The ability to use initiative, think innovatively and continually look for ways to meet client needs and desires.
Sensitivity to the needs of older people of different social, economic and cultural backgrounds.
Experience and/or knowledge of Home Care Packages

The salary band for this position is between $68,675 & $73,800, dependent on qualifications and experience.
