Description:
Promote the values of CORE to provide clients with independence, participation, care, self-fulfilment and dignity. Ensure awareness and support of the philosophies of healthy ageing and independent living and upholding the rights of clients.

The role primarily focuses on providing Early Intervention Case Work and support to young people, predominantly in the 12-18 age range, who have complex needs, who are homeless or at risk of homelessness across the South Western Sydney Area District.

The position will provide this support in young people’s environments, to improve their access to homelessness services and support initiatives, education & training, living skills training and links to broader health and mental health, welfare, child protection and alcohol and other drug services.

The position will liaise with and coordinate support from agencies that offer relevant services to the clients group and their families/ carer givers.

The position requires a commitment to organizational values and the achievement of strategic and business objectives. All work undertaken will reflect CORE Community Services Mission and Values.

The role will require the incumbent to initiate new community partners to establish outreach and training programs.

Responsibilities include:

  • Undertake and meet all case management responsibilities in relation to young people allocated by the Senior Case Worker. Casework with a manageable caseload
  • Maintain regular, effective and meaningful contact with young people.
  • Work within a client-centred, trauma-informed framework that is culturally appropriate.
  • Ensure that case plans are regularly reviewed and facilitate the attendance of all key stakeholders at relevant meetings including case conferences and reviews.
  • Attend regular meetings from each service area in Youth Services to provide culturally appropriate services for clients, and gauge culturally appropriate training needs. Identify personal learning needs and give feedback on the service including the identification of gaps and areas for improvement.
  • Provide clear and concise advice, assistance and/ or recommendation strategies to Young People and their families in South West Sydney
  • Actively marketing and promoting CORE Youth Services programs and activities to new prospective participants and service providers. 
  • Develop and maintain positive and effective professional working relationship with internal and external stakeholders.
  • Work collaboratively with Team Leaders, Case Managers, Residential Care Workers, administration staff and other Core Community Service staff to achieve identified outcomes for young people.
  • Undertake case management including reviews, reporting and administration functions.
  • Work collaboratively with staff to facilitate group work activities in line with the Targeted Early Intervention (TEI) Outcomes Framework.
Skills and Experiences:
Selection Criteria:
  • Relevant degree qualifications in Social Work, Psychology or equivalent
  • Demonstrated experience in Case management from a strengths-based, trauma-informed and family-focused framework
  • Awareness of child protection issues, with the capacity to identify levels of risk requiring statutory child protection intervention.
  • Knowledge of the Children and Young Persons (Care and Protection) Act 1998
  • Superior case management skills and previous experience in a similar role
Demonstrated understanding of the issues faced by young people and an ability to advocate for social justice outcomes for the communities that you represent.

  • Drivers Licence
Strong communication, customer service, negotiation and influencing skills. Including resilience, with the ability to work under pressure and manage high volume workloads.

Demonstrated dedication to work within professional boundaries, integrity and ethical standards.

Demonstrated sensitivity to Culturally and Linguistically Diverse (CALD) individuals and communities.

Demonstrated experience in facilitating group work activities.
