About the role


The GEM@Home Program service is a ‘bed substitution/diversion model,’ replicating GEM-bed based services in the client’s own home. It is an inter professional model of care with 7 days a week service delivery .The interdisciplinary team includes Geriatricians , Nursing, Allied Health, Allied Health Assistant , Pharmacy , Personal and Domestic assistance services (via brokerage) delivering care.


 GEM@Home provides care for the person who still needs assistance with daily tasks for conditions associated with ageing, cognitive dysfunction and chronic illness. Patients admitted to the program have access to diagnostic and management services, including care coordination. The average length of stay for patients is approximately 2 weeks; the program assists clients to achieve their goals-of-care in their own home.


 The GEM@Home Coordinator additional responsibilities related to the program will include:



  • Coordinating delivery of the program.



  • Rostering of a 7 day a week service

  • Recruiting and orienting new staff members to the program

  • Advising all staff on professional development activity within GEM@Home

  • Coordinating the staff performance feedback and development process.

  • Engaging all staff in the development of the team


    • Coordinating clinical-based team meetings that enable communication across the team;

    • Organizing professional development for the team, based on a needs analysis; 

    • Providing and coordinating specialty clinical support for clinicians and managers to ensure consistent service;

    • Advocating for clinical-specific issues that may arise

    • Advising the GEM@Home Manger on clinical equipment requirements and professional development needs

    • Informing the Manager of any circumstances, incident, issue or event which may have an impact on the operation of the program or its outcomes.



About you


To be successful in this role you will possess:



  • Current registration with AHPRA 

  • Post Graduate qualifications in the relevant clinical or academic field or working towards

  • Highly developed interpersonal and leadership experience

  • A strong commitment to high quality care

  • The ability to work as part of a diverse team

  • Excellent communication and organisational skills

  • Current Driver’s license
