The Organisation

Our customer is a leading health and family - based service providing a holistic approach to the wellbeing and needs of the Aboriginal and Torres Strait Islander community in Brisbane and surrounding areas. Our customer is dedicated to providing quality health care around key areas such as GP services, dental, mental health, child/family and maternal health, optometry, podiatry, dietary and other primary health care needs. In addition to this, the organisation provides quality intervention services to youths and families in the early intervention and out of home care sectors.

The Role

Based in Woolloongabba and/or Springwood, The Senior Youth Practitioner provides tailored case management services for young people at risk of disconnecting with their family / community, disengagement from education, training and employment, homelessness, or at risk of harm. Youth Practitioners are responsible for effective engagement with young people, their families and their support network to identify the young person’s needs and aspirations, and record and progress a plan created and led by the young person. The Senior Youth Practitioners are the main point of contact for the young person and their family, and work in partnership to facilitate supports required to achieve their goals through coordination of a wrap-around service. These positions are based in Woolloongabba and Springwood.

Key Responsibilities

The Senior Youth Practitioner is experienced and has proven high performing case management experience who also assist the Youth Manager in oversight of service delivery and continuous improvement.

Senior Youth practitioners develop, implement and execute the care plan of young people and coordinate a holistic approach to achieve the following program objectives:

  • Connect to positive family support
  • Get a job or engage in training or education
  • Lead a healthy and violence free life
  • Have a safe, stable place to live

To Be Successful

The successful applicant will possess the following:


Qualification and/or Experience:

Current Bachelor’s Degree, or actively studying, degree in social work, Human Services, Social Science, Health, Allied Health, Youth Work, Community Services, Family Intervention, or 5 relevant years’ experience in field.

Minimum of three years’ experience in case management and coordination activities using a client information system, as part of the 5 years’ field experience or in addition.

Skills and Knowledge:

  • Demonstrated ability and comfortableness working in a performance driven environment to meet individual weekly, monthly and quarterly performance targets.
  • Demonstrated ability to build client agency, self-efficacy and facilitate change through client empowerment, while actively discouraging client dependence on supports.
  • Demonstrated knowledge and ability to provide behaviour change interventions that build client self-awareness and active change in achieving their identified goals.
  • Demonstrated ability to work with the whole-of-family, irrespective of primary client, in meeting the totality of family needs.
  • Demonstrated ability in coordinating multiple services in providing access to holistic care for clients, particularly integrating internal agency supports across multiple teams.
  • Demonstrated knowledge and understanding of the factors contributing to vulnerability, disengagement, and intergenerational trauma of client group
  • Experience with case management for high risk and/or crisis clients, with high-level planning and organisational skills.
  • Knowledge of the supports available to target clients in the areas of mental health, substance misuse, education, employment and housing.

Additional Requirements:

  • Requirement to travel across the Logan and/or Brisbane region, as per program location.
  • Current drivers’ licence
  • Requirement to work outside core 9am-5pm business hours in meeting individual client needs and provide responsive care.
  • Working with Children’s Card (Blue Card) or be eligible to apply
  • Requirement to work 1-2 night outreach shifts a week, when required.

Salary and Benefits

Salary for this position will commence at $77 567 plus salary sacrifice and super. This position also involved 17.5% leave loading and pool vehicles available for work use.
