Transition Support Mentor

The Transition Support Mentor is responsible for working creatively and resourcefully with participating organisations to provide training and support to enable them to assist their participants to submit high quality NDIS access requests. The successful applicant will assist with the development of training resources and deliver training and support to the participating organisation in regional and remote locations.

Essential Criteria:

  • An understanding and knowledge of mental health issues as they pertain to people with severe and persistent mental health issues;
  • Extensive understanding of the NDIS access criteria;
  • Highly developed interpersonal, communication, liaison and advocacy skills;
  • Previous experience in community development and mental health;
  • Previous case management experience;
  • Excellent organisation, time management and prioritising skills;
  • Demonstrated problem solving and conflict resolution skills;
  • An awesome work ethic, which includes attention to detail;
  • Outstanding written and verbal communication skills;
  • Proficient administrative skills and knowledge with experience in the Microsoft Office Suite;
  • Willingness to travel within WA if required.


  • Qualification in mental health or a relevant area;
  • Current Australian Drivers Licence with no restrictions;
  • Current National Police Clearance Certificate with 6 months or less currency;
  • Current Provide First Aid Certificate with 12 months or less currency;
  • Unlimited rights to work in Australia.


  • Full-time contract until 30 June 2021, with possibility of extension;
  • Work from home or one of our many offices in the Perth metro, Mandurah, Albany or Bunbury. This role is only open to residents of WA due to the requirement to occasionally travel within WA;
