Our client, 4LifeSkills is a not for profit community organisation based in the southern suburbs of Perth delivering a range of quality services to people with disabilities to maintain and enhance a quality of life that is meaningful to them.

About the role:

We are looking for a qualified, enthusiastic and compassionate person to work with individuals and their families to understand, implement and use their NDIS support plan. You will supervise and mentor Support Coordinators and offer leadership skills and knowledge to the team, whilst working with individuals and their families to ensure a mix of supports are used to increase capacity to maintain relationships, manage service delivery tasks, live more independently and be included in the community.

Duties will include:

  • Supervision of the Support Coordination team
  • Allocating clients to Support Coordinator case loads in line with capacity and skills
  • Building relationships and partnerships with external stakeholders to ensure the needs of clients are met
  • Guiding Support Coordinators to deliver services that meet client needs
  • Ensuring Support Coordinators submitting accurate and timely reports
  • Supporting clients to successfully implement their plans
  • Participating in the recruitment and training of new Support Coordinators
  • Attending meetings with prospective clients to promote the service
  • Monitoring plan outcomes and expenditure to ensure plans remain within funding budget
  • Providing all services within the legislative framework


  • Cert IV Disability Services - Minimum
  • Demonstrated supervisory experience
  • Completion of Support Coordination course and have the certification
  • Sound knowledge of disabilities and a modern contemporary view of support services for people with a disability in the community
  • Recent experience in the disability services sector, including NDIS
  • Demonstrated experience in the provision of support coordination for people with disability
  • Demonstrated experience in developing good relationships within the disability sector
  • Demonstrated ability to organise and prioritise tasks, work effectively under pressure, problem solve and meet deadlines
  • Ability to work with minimal supervision and as part of a team
  • Intermediate computer skills
  • Excellent communication skills, written and verbal
  • Current National Police Clearance (less than 6 months)
  • Current First Aid and CPR Certificate
  • Current Working with Children check
  • Current "C" class drivers licence
