About the Opportunity

  • To support in the implementation and adaption of the PCLI service model within MHDA.
  • To support other MHDA clinicians to assess, plan, implement and evaluate recovery orientated and person centred care co-ordination interventions targeting mental health consumers experiencing severe and persistent mental health conditions. 
  • Utilise recovery principles for long stay consumers and consumers at risk of a long admissions, in a hospital or community setting.
  • To assist with links in the community for consumers with complex mental health needs, this includes: NDIS providers, NGO's and providers of therapeutic interventions as required to meet consumer support and recovery needs, as part of their individual collaborative care plan.
  • To identify the needs and provide education, support and assistance to families and carers and other community stakeholders (capacity build).
  • To fulfil in a timely manner all health service reporting requirements including data collection and outcome measures.
