As part of the Civitas Group, Supported Care is a privately owned organisation designed to provide support services to individuals living in SILs, level 3 supported accommodation and the community. Our services are funded through NDIS services charges.

About the Role - Multiple Positions Available!
Reporting to the Team Leader, the successful candidates will be responsible for providing residential and community customers with assistance, support and encouragement of independence to reside in their chosen community. You will strive to meet the physical, social, intellectual, psychological and emotional needs of the customers. We are seeking eager candidates to fill multiple positions across our sites
The Support Worker is an integral member of Supported Care team
Position Responsibilities
- Provide Community Participation to residents by providing transport, assisting them on social outings, shopping etc;
- Provide general Household Tasks; undertake housekeeping and laundry duties;
- Participate in the review of care plans by ensuring all necessary documentation is complete and accurate; report any observed changes to residents health to the Team Leader
- Perform position responsibilities in accordance with the Person Centred Model of Care.
The Ideal Candidate
The ideal candidate will have compassion and experience working with those facing various life challenges, including interacting with people living with mental illness and disabilities.
The ideal candidate will possess:
- Current driver’s licence and a willingness to use your own comprehensively insured vehicle for work purposes (preferred);
- Current Senior First Aid Certificate and Cardiopulmonary Resuscitation (CPR) certification or ability to obtain;
- Ability to read, write and communicate effectively in English;
- Smart phone to use rostering App;
- Certificate III in Individual Support (Home and Community) or Certificate III in Disability Support or equivalent;
- Basic computer skills, including the ability to use Outlook and Internet Explorer;
- Possess a current Yellow Card – or the ability to acquire;
- A polite and friendly manner;
- Proficient time management skills;
- High personal, professional and ethical standards.
