Care Coordinator Clinical Team Leader
Sub-Acute Non Admitted
Grade 4A
Fixed Term Full or Part Time
(up to 80 hrs per fortnight) with ADO
The Position
We are seeking a suitably qualified Registered Nurse to join our professional Sub-Acute Non Admitted team. This fixed term full or part time position is available from mid February 2021 and includes working Monday to Friday. This role involves leadership, care coordination, assessment, education and responsible for clinical day to day coordination of the team. The successful applicant will be responsible for deputising for the Manager of Sub Acute Services and duties as delegated.

The Health Service
Wimmera Health Care Group has an average of 325 births per year, 12,500 admissions and 4,600 operations and 16,000 ED attendances annually. We have a modern theatre complex, including a day procedure unit, a five-bed intensive care unit, and acute and subacute wards all set within a picturesque garden setting.
Key Selection Criteria include:
Essential:
- Tertiary qualification in nursing with current registration and authorisation to practise
- Current drivers licence
- Clinical knowledge and experience relevant to chronic disease
- Care Coordination and advanced assessment skills
- Demonstrated and ongoing commitment to professional development
- Advanced development of excellent communication, problem solving and time management skills, leadership and coordination skills, organisational and problem solving skills
- Understanding of chronic disease management and self-management principles
- Understanding of sub-acute non admitted health independence programs and services
- Computer literacy
Desirable:
- Previous experience in Team Leader role
- Experience working in a community based setting
- Knowledge of community support services
- Skills in motivational interviewing
- Demonstrated ongoing commitment to professional development
