This is a Recruitment Pool for Permanent Full Time, Permanent Part Time, Fixed Term Full Time and Fixed Term Part Time appointment with the possibility of extension(s) and/or permanency.

Position Profile:

Service Commissioning Phase
In collaboration with the Clinical Nurse Consultant, the Care Navigator will assist with service design and resource development to ensure service readiness, providing information to the health sector as well as involvement in community awareness as appropriate.

Service Operations Phase
The role works in collaboration with a person’s existing services and local providers to ensure coordinated service delivery, provision of appropriate information and support in the area of Voluntary Assisted Dying (VAD). Successful candidates will act as a role model and consultant to other health service providers (in both the Perth metropolitan and regional areas), providing knowledge and clinical expertise in the area of VAD, collecting and recording data and information as required by the service. 

Area Profile: The South Metropolitan Health Service (SMHS) delivers quality, safe and effective hospital and health services within a catchment area stretching more than 3300 square kilometres across the southern half of Perth.
