About the Role
The purpose of this role is to design the establishment, implementation and monitoring of the Frail Older Persons Program across Central West Hospital and Health Service. This includes the ability to deliver consistent and effective clinical innovation practices and redesign processes to deliver solutions that are responsive to customer needs, sustainable and enhance health services for this demographic.

This role will be accountable for the design of strategies that are measurable and focus on high volume, high impact outcomes that are co-designed to meet the needs of community.

Salary Information
Total remuneration value of up to $11,984p.a.; is comprised of salary between $116,902- $126,037p.a.; Nurse Grade 7; temporary full time; employer contribution to superannuation (up to 12.75%); annual leave loading (17.5%); accommodation assistance; applications will remain current for 12 months.
