Job Description

About The Role

As a Clinical Care Manager - Operations Support, you will manage and lead the delivery of person-centred care of residents, ensuring the highest standard of ongoing assessment, care planning and evaluation and clinical governance, helping them to live longer, healthier and happier lives.

Travel is essential in this role, as you will be required to travel across care home.

This is a 12 month full-time fixed term contract.

Duties

  • Maintain overall accountability and responsibility for the clinical management of health and care outcomes of the residents in the care home
  • Ensure the delivery of quality Clinical and Person-First assessment for all residents in the care home
  • Oversee the coordination of ACFI with collaboration of the GM
  • Deliver highest quality clinical care to residents – including pre-admissions, documentation, care plans, referrals, infection control
  • Manage medication inclusive of Schedule 8 drugs in accordance with resident needs and legislative requirements
  • Lead the Care Home team in the delivery of person-centred care
  • Manage person centred care planning and evaluation for residents

Skills And Experience
  • Bachelor of Nursing or Equivalent is essential
  • Post grad qualification in aged care highly regarded
  • Current AHPRA is essential
  • Proven track record in delivering quality clinical care
  • Excellent clinical assessment skills
  • Strong leadership and care planning capabilities
  • Highly effective communication and relationship management skills
  • An understanding of Resident Rights, Aged Care Accreditation Standards and Outcomes
