Sydney Local Health District (SLHD) is seeking a dynamic Community Mental Health Clinician to join our team at Croydon Health Centre.

Do you have tertiary qualifications in occupational therapy, social work, psychology or nursing. Full AHPRA registration (OT, Psychology & CNS2), or eligibility for membership to AASW (SW). A minimum of three (3) years full time equivalent experience in mental health. Approved post registration qualifications deemed appropriate by the organization for CNS2 grading?

Have you demonstrated advanced skills in the comprehensive assessment and management of complex mental health consumers, including ability to prioritise and problems solve clinical issues, assessment, care planning & coordination, service linkage, knowledge of harm minimisation and discharge planning?

This position will provide senior clinical leadership within mental health care coordination focussed on long stay complex consumers discharged to community, and including optimisation of consumer care delivery, and supporting the mentoring and development of staff.

This position is a community based clinician that will support integrated mental health care across inpatient and community sites. Based in a Community Mental Health care coordination team, this position will work within community mental health services to support long stay mental health consumers ensure community tenure, and ensure our accountabilities for follow up and reporting against the PCLI program to the Ministry are met. The role involves health promotion, assessment, brief intervention, care-planning, linkage and referal to support complex consumers needs. This position will work closely with a range on internal (for example Drug Health) and external (community managed organisations including HASI, the NDIS, primary care) partners to ensure effective care.

The Senior Clinician has three primary roles. Firstly, this position will develop and provide direct clinical interventions for a reduced care coordination case load, likely to be consumers who have met the PCLI criteria in the Care Coordination team the position is based. Secondly, this position will support workforce skill development enabling staff to better support people with complex mental health presentations and ensure community tenure. Our experience with this part of the role includes sourcing additional community based supports during times of increased psychosocial need, negotiating with the NDIS, negotiating with housing providers to ensure stable housing during crisis, and supporting integration into community supports. Thirdly, the position will have accountability for ensuring the Pathways to Community Living (PCLI) assessments and reporting are completed, as directed.

To be considered for this role you must have…

  • Current unrestricted NSW drivers licence (P2 licence acceptable)

  • The ability to work Monday to Friday 08:30-17:00
