Position Summary

The Community Mental Health Enrolled Nurse is part of the multidisciplinary Wellness Clinic and is responsible for delivering physical health assessments, interventions, monitoring and referrals for clients of the Adult Community Mental Health team. The EN will see clients both in the clinic and by assertive outreach in the community. The EN works competently within their scope of practice to deliver safe, evidence based person centred care to achieve optimal physical health outcomes.

The Mornington Adult Case Management Team provides community treatment through recovery focused case management for clients 25-64 years. The team works Monday - Friday 8:30am-5pm.

Key Responsibilities

• Responsible for the Physical Health Monitoring in collaboration with the multidiscipline team as part of the Wellness Clinic
• Assess, plan, implement, evaluate and document comprehensive Physical Health assessments in accordance with evidence based best practice standards and Peninsula Health Values.
• Work competently within scope of practice to deliver safe, evidence based person centred care to achieve optimal Physical health outcomes.
• Participate in the running of wellness groups in collaboration with the Wellness Clinic team utilising expertise in regards to physical health interventions and outcomes.
• Work under the supervision of an RPN and operational governance from Team Manager.
• Assist clients in achieving their physical health recovery goals and providing support to case management clinicians in care delivery to the client
• Maintain data on the number of clients receiving physical health assessments and the outcomes and report on KPIs as required
• Collaborate with multidisciplinary team of both the Wellness Clinic and the Adult Community Mental Health Team
• Establish relationships with key internal and external stakeholders
• Assist with the administration of medication as required
• Escalation of clinical and safety issues to senior staff
