Have the option to provide a high standard of service delivery either face to face or from the comfort of your own home as you collaborate with allied health professionals to find best practice supports to clients in this multidisciplinary practice

As a key contributor you will be responsible for:

  • Providing high quality support to clients with impairment due to illness, injury, disability or ageing
  • Assess the functional abilities of the client utilising a range of assessment techniques and tools
  • Create and assess holistic plans that are outcomes driven
  • Assist in the implementation of plans
  • Advise and collaborate on home alterations
  • Establish and maintain relationships with stakeholders to ensure best practice for holistic client focused care
  • Produce accurate client reports, files and communications to other health professionals.
  • Work effectively with other professionals, as part of a virtual multi-disciplinary team
  • Provide support and guidance to other team members as required ensuring the delivery of high quality services for clients.
  • Participate in quality assurance activities that contribute to improving service and client experience

  • Manage caseloads, prioritising needs and completing administrative tasks
  • Be competent in the use of technology and online platforms

Qualification Requirements:

  • Bachelor or Master’s degree in Occupational Therapy
  • AHPRA Registration
  • A current cleared Police report
  • Relevant clearance to work with Vulnerable Persons
  • Valid Working with Children Check
  • Access to a roadworthy and comprehensively insured vehicle
